Unpaid balances, including all applicable co-payments, co-insurance, deductibles and any non-covered services are the responsibility of the patient and must be paid within 30 days of receipt of the statement. Payments may be made via

We will make available, under the Community Care Program, a reasonable volume of service at no cost or less than full charges to persons meeting the criteria. Eligibility for Community Care may be limited to persons whose family income is not more than 300% of the current poverty income guidelines established by the Department of Health and Human Services.
